Retrograde coronary sinus perfusion with cold potassium blood cardioplegia

Abstract
Myocardial protection during aortic cross-clamp was performed using retrograde coronary sinus perfusion with cold potassium blood cardioplegia in 12 clinical cases which were compared with integrate coronary sinus perfusion in 26 clinical cases. The retrograde coronary sinus perfusion with cardioplegia was indicated when the aortomy was required in open heart surgery such as aortic valve replacement and when coronary artery bypass surgery was performed in myocardial infarction. The combination of this method and topical cooling with ice slush yielded promptly hypothermia of myocardium. The temperature of the interventricular septum was maintained below 20¡ÆC by continuous perfusion or intermittent perfusion of cold potassium blood cardioplegia.
This technique provides clear operative field and avoids some serious complications which are caused by coronary postal cancellation.
We investigated 1) the myocardial temperature during operation 2) the enzymes activity during operation and after operation as an evaluation of myocardial ischemic injury, 3) isotropic support after operation and 4) arrhythmia after operation.
From these results, we concluded the retrograde coronary sinus perfusion with cold potassium blood cardioplegia is one of the outstanding procedures of heart protection during the aortomy.
